Your selected form can be procured from the Fulfillment House. The Fulfillment House is the University's central repository from which departments can order forms and publications online through K/P Corporation. It is fast and easy, and there is no cost to departments. An order is usually processed within 72 hours.
To access the Fulfillment House, each department will need a user ID number and password, provided by the Benefits Office (as described in the instructions below).
| To set up a Fulfillment House account: | ||
| · | contact the UCR Benefits Office at extension 2-4766 to procure your user ID and password | |
| · | go to the web site: http://apps.kpcorp.com/ucbenefits | |
| · | enter your user ID and password (note: the user ID and the password are the same) | |
| · | type your one-time shipping information | |
| To place an order: | ||
| · | click on "Order Materials" and make your selection | |
| · | you can preview your order by clicking on "Preview Order" | |
| · | to make changes, click on "Edit Order" | |
| · | once your order is correct, click on "Submit Order" | |
| · | retain the Order ID for future reference | |
| · | you will receive an e-mail from K/P Corporation as to when your order will be shipped | |
| To check the status of an order: | ||
| · | go to the Main Menu, click on "Check Order Status" | |
| · | from there you can either enter your Order ID or you can see a list of all the orders you have placed | |
| · | make your choice and then click "Go" to view the detailed order and shipment information | |
